One of the best format guidelines for comics is from Dark Horse Comics. It’s a very good reference for what publishers like to see from a submission.

For script examples I recommend the books Panel One and Panel Two. They’re collections of scripts from industry leaders.

For an excellent view on capturing the big picture I recommend Alan Moore’s Writing for Comics.

For a detail oriented look at scripting for a monthly series there is the DC Guide to Writing Comics. It has a great method for tracking multiple sub plots.

For an example of a script and the final product all in one book try Arkham Asylum, 15th Anniversary edition. It starts with the full graphic novel and then ends with a copy of the actual script and notes from the writer about the process. Note, only the anniversary edition has the script at the back.

This one comes highly recommended to me. I haven't read it yet but it is next on my list.
